• No appointments necessary.
• Schedules are based on vaccine availability

The Lake Square Mall immunization site is available by appointment. Residents may preregister for an appointment online atmyvaccine.fl.gov or by phone at 866-201-6909 (or 833-476-1031 for TTY users).  As a reminder, the last day to receive your first dose at this site will be Tuesday, May 4^th.

All Florida residents are now eligible to receive any COVID-19 vaccine as prescribed by the Food and Drug Administration.  The Pfizer vaccine is authorized for persons 16 and up (all minors must be accompanied by a parent/legal guardian).  The Moderna and Janssen (Johnson and Johnson) vaccines are authorized for persons age 18 and up.
