To help parents and guardians prepare students to go back to school, the Florida Department of Health in Lake County (DOH-Lake) will be extending their immunization clinic hours on July 26 till 6:30 pm at the following locations:

Vital Statistics will also be on site at this location for birth certificate services.

Immunization services are offered Monday through Fridays at the DOH-Lake locations listed above.  Please call for hours and appointments.

There is no charge for immunization for children, birth to 18 years of age provided they are still enrolled in elementary, middle or high school.  Parents or guardian must bring a copy of the child’s immunization records.  School physical examinations are also offered by appointment only.
