Building Blocks Ministries’ was recently awarded a grant from the Lake County Board of County Commissioners Human Services grant program FY 2018-19 for their project, “Conferences, Training and Targeted Case Management. The objective of the project is to help individuals with developmental disabilities and families connect with representatives from state agencies to obtain the services and supports they need most. In an effort to connect individuals and families to services Building Blocks Ministries will be hosting four conferences for the community. Each conference will include presentations from the Agency for Persons with Disabilities, Vocational Rehabilitation and the Family Care Council.

The Agency for Persons with Disabilities (APD) is the agency who determines if an individual is eligible for services under the iBudget Home and Community Based Services Waiver. This waiver helps the individual and their families pay for services such as; day training, transportation, group homes, personal supports, and more. The process begins with an initial questioner form that must be completed and submitted to the Agency. Representatives from APD’s Wildwood Office will be available at the conferences to assist individuals and families. Building Blocks’ Case Manager will also be available to assist and will provide support as the families walk through the application process. A Supported Employment Liaison from APD will also share about services and support available for individuals who are interested in working in the community.

Representatives from the Florida Department of Education Division of Vocational Rehabilitation, Winter Garden office, will present information about Preplacement Training, Self-Advocacy and Supported Employment. They will also provide referral forms for interested individuals to complete to register for services.

Conferences times are 9:00 am to noon and will be held at the Community Foundation of South Lake, located at 2150 Oakley Seaver Dr., Clermont. All conferences are free and open to the community.
